Supported Independent Living

Forte Care Services offers Supported Independent Living (SIL) to assist individuals with or supervise their daily tasks, fostering the development of skills for maximum independence. These services encompass various forms of support provided within the participant's residence, regardless of property ownership, and can be tailored to both shared or individual living arrangements.

Our seasoned staff facilitate the pursuit of interests and hobbies, foster social connections, plan outings, and encourage community engagement. Whether residing independently or with fellow individuals with disabilities, we provide assistance in maintaining a comfortable living environment and managing day-to-day household chores such as meal preparation, laundry, or tending to a garden.

Social, Community and Home Care

This service provides NDIS participants with the necessary support and assistance to actively participate in their community. This may involve engaging in social and recreational activities either individually or as part of a group. Forte Care Service's social, community, and home care service extends to include additional support aimed at enabling participants to attend Centre-based programs.

These programs are designed to equip individuals with skills essential for independent community access and engagement. Furthermore, our services encompass personal care and domestic assistance, covering tasks such as shopping, cooking, cleaning, recreational activities, and transportation.

Respite Care (STA) & Medium Term Accomodation

Respite services, also known as Short Term Accommodation, offer planned breaks for both NDIS and non-NDIS participants, providing temporary relief from the family home. These breaks aim to support the ongoing relationships among family members and unpaid carers of NDIS participants, with the understanding that families and carers will resume care after the respite period.

Short Term Accommodation is provided in a designated 'respite' house, where participants stay overnight or for a specified duration. The advantages of Short Term Accommodation for participants include the opportunity to step away from their regular routines, engage with new people, and enjoy a supportive communal living environment. Additionally, respite offers carers much-needed breaks, allowing them to recharge and return refreshed when reconnecting with their clients.

Transportation

Linking you to vital services and your community, Forte Care Services offers transportation solutions designed to simplify your journey to various destinations, be it for grocery shopping, medical appointments, or any other outings.

Our transportation service facilitates tasks requiring you to leave your home, ensuring a smooth and hassle-free journey to your intended destination. Positive Behaviour Support

  1. Positive Behaviour Support enhances your quality of life through the development of new support strategies, skills, and behaviors tailored to your needs and aspirations.

    We collaborate with you and your team to create a personalized Plan, founded on an understanding of:

    • Your current routines, values, social connections, and personal strengths
    • Any challenging behaviors you may be experiencing presently
    • Your needs, exploring alternative ways to address them without resorting to challenging behavior
    • The positive behaviors and skills you aspire to cultivate in order to achieve your meaningful goals

  2. Forte Care Services can support participants by developing and implementing individualized behavior support plans, providing ongoing assessment and analysis of behaviors, and offering training and guidance to participants and their families. Forte care services positive behaviour practitioner works closely with participants to:

    • Identify triggers and develop strategies to manage challenging behaviors
    • Promote positive behaviors and enhance overall quality of life
    • Ensure consistent monitoring and adjustments to the behavior plans as needed, ensuring that the support remains effective and responsive to the participant’s evolving needs

Support Coordination

  1. We provide support coordination services to help individuals navigate the National Disability Insurance Scheme (NDIS) and access the supports and services they need to achieve their goals.

  2. At Forte Care Services, we assist a participant with support coordination by helping them navigate and organize their various services, ensuring that all aspects of their care are integrated and aligned with their goals. This includes assisting in selecting and managing service providers, facilitating communication between different support services, and advocating for the participant’s needs to ensure they have choice and control over their care. Through ongoing guidance and support, the participant can make informed decisions and effectively manage their overall support plan.

  3. Forte Care Services can assist a participant with support coordination by conducting personalized assessments to understand their needs, helping them select and manage service providers, facilitating effective communication between all parties involved, advocating for the participant’s preferences, regularly monitoring and reviewing their support plan, and empowering them with information to make informed decisions and maintain control over their care.

  4. At Forte Care Services, our support coordinators assist participants in building the skills needed to understand, implement, and utilize their plans effectively. We work with participants to ensure they use a variety of supports to increase their capacity to maintain relationships, manage service delivery tasks, live more independently, and be included in the community. Our coordinators help you understand and implement the funded supports in your plan, support you in achieving your goals, and build your independence to live your life the way you want. Additionally, our coordinators assist you in liaising with providers about the services they offer and support you in your everyday life.
